Well Formed Desired Outcomes (Goals)
Goals are something that you want to achieve. Outcomes are something that you create by achieving your goals.

1.What is your goal(s) in a SPECIFIC context? What do you want/desire?
2.Short term or long term? Remember to break down your goal into SMALL steps.
3.Is your outcome stated in positive terms?
4.Are you saying what you want or what you do not want? (only state what you want)
5.Is this goal initiated and controlled by you? If not, then who else is involved?
6.Is this a realistic goal?
7.Can you achieve this yourself or do you require help? If you require help, from what resource?
8.What will having this outcome get for you?
9.How do you know when you have your outcome
10.Will your evidence procedure provide you with measurable feedback?
11.Where, when and with whom do you want this?
12.How will your desired outcome affect every area of your life? (i.e. family, work, etc.)
13.What resources do you have already (internal and external) that can be used to get your desired outcome?
14.Do you need additional resources to get your outcomes? If yes, what.
15.What process has to occur to reach your outcome?
16.How long will it take to reach your outcome?
